(1) Vendor needs to provide strengthen and coordinate services to the government authority located in Ann Arbor, MI. – Health and Wellbeing:• Education programs and classes.• Healthcare access and coordination.• Disease awareness, education and prevention.• Physical, mental, and behavioral health.– Community Living Long–term Services and Supports:• Insurance and long–term services and supports options counseling.• In–home care for those on waitlists for long–term services and supports.• Friendly caller programs.• Adult Day Programs.– Care Partner Support:• Adult day and respite services.• Education and support groups.– Care Management:• Care partner comprehensive care management.• Older adult comprehensive care management.• Interdisciplinary and collaborative care management models.– Legal Services:• Advance directives for health care.• Designation of patient advocate.• Guardianship.• Estate planning.• Elder abuse, neglect, and exploitation legal services.(2) All the questions must be submitted no later than July 27, 2026.
