Vendor needs to provide primary care community health needs assessment service to the government authority located in Juneau, AK.    – To conduct a statewide community health needs assessment to identify communities across the state experiencing the greatest unmet health care needs, outcome disparities, and workforce shortages; poverty; and barriers to accessing care.              • Unmet needs amongst Alaskans and their unmet primary, dental, and mental health needs • A discussion of any relevant barriers, by service area • A plan for collaboration with recipients or interested parties • Identify underserved target populations and, in coordination with the Primary Care Office Director, suggest implementation strategies to improve health outcomes and strengthen partnerships
