First 5 Sacramento: Help Me Grow Model: Developmental and Behavioral Screenings, and Centralized Referrals

The National Help Me Grow (HMG) Model provides a comprehensive approach to early childhood prevention and intervention through four pillars: 1) Increasing early identification of delays through developmental and behavioral screenings, 2) Educating families, healthcare providers and the community about the importance of early identification and intervention, 3) Connecting families to services and supports, and 4) Reviewing data to identify gaps and opportunities for improvement. Focus of Funding : Implement and Strengthen the HMG Model Amount of Funding Available: A maximum of $1,020,000 is available cumulatively over the three-year period, July 1, 2024 – June 30, 2027.
