AI-1039 - USA (Connecticut) - AI-Enabled HealthCare Coordination and Remote Patient Monitoring Service - Deadline July 7,2026

(1) Vendor needs to provide health care coordination and remote patient monitoring using artificial intelligence (AI) service.        – Remote patient monitoring (RPM), combined with AI–enabled care management and assistive technologies, represents a key strategy for improving access, quality, and continuity of care in rural communities where geographic barriers and workforce constraints limit timely access to services.– Expand access to care through technology–enabled delivery models, including AI–powered RPM, virtual care, and home– and community–based services that help address rural workforce shortages.– Promote long–term sustainability by supporting adoption of Medicaid and commercial reim–bursement pathways and strengthening the financial stability of rural providers through strategic technology investments.– Operational capability to implement and support AI–enabled health care delivery tools in clinical settings.– Municipal health care providers or public health agencies delivering clinical services.– Emergency medical services (EMS) and community paramedicine.– Behavioral health and substance use disorder treatment providers– Pharmacists and pharmacist–led clinical services.– Hospitals and health systems.– Independent medical practices.(2) All the questions must be submitted no later than June 01, 2026.         (3) The contract period will be for one year.
