Early Intervention Services for Older Adults

General work description: The County of Orange (County) Health Care Agency (HCA) is seeking proposals from qualified organizations to provide Early Intervention Services for Older Adults (EISOA). The primary goal of this program is to increase access to mental health treatment services and supports through linking eligible, older adults (including monolingual non-English speakers) to behavioral health services, providing case management, and socialization supports to reduce risk factors such as decreased physical activity, isolation, and misuse of substances while increasing protective factors. The program will focus on those diverse unserved or underserved individuals who have not sought out services due to the stigma associated with mental illness and behavioral health conditions and/or cultural/linguistic barriers. Additionally, the program will help those with mild to moderate conditions get linked to a managed care plan when appropriate services are available.
