Description

PCHS is seeking proposals from eligible and qualified organizations to provide an evidence-based Alzheimer’s/dementia intervention to support family and other unpaid caregivers caring for an aging parent, ailing spouse, or other adult with Alzheimer’s disease or other forms of dementia, AND/OR Older Adult Counseling Services, Family Caregiver Counseling Services and an in-person Caregiver Support Group for family and other unpaid caregivers caring for an aging parent, ailing spouse, or related adult with a functional disability that meets at least once per month. Federal Older American's Act (OAA) Title IIIE Funds and Washington State Family Caregiver Support Funds are allocated for implementing Family Caregiver Support Program Evidence-Based Alzheimer’s/Dementia Interventions. Aging and Disability Resources (ADR) Case Managers will screen, authorize, and refer eligible family caregivers to the contracted agency for the evidence-based Alzheimer’s/Dementia intervention through the CLC-GetCare database. Pierce County Human Services has made available the following funds: Older Adult Counseling (OAC) Up to $150,000 annually from Federal Older Americans Act (OAA) Title III B (ALN 93.044) funding. Up to $9,000 annually from Federal Older Americans Act (OAA) Title III D (ALN 93.043) funding. Family Caregiver Counseling and Caregiver Support Group (FCC/CSG) Up to $12,400 annually from Federal Older Americans Act (OAA) Title III E (ALN 93.052) funds and Washington State Family Caregiver Support Program (SFCSP) funds. FCSP Evidence-Based Alzheimer’s/Dementia Interventions (ADI) Up to $20,720 annually from Washington State Family Caregiver Support Program (SFCSP) funds Up to $2,100 annually from Federal Older Americans Act (OAA) Title III E (CFDA 93.052) funds. OAA Title III B and Title III E funded programs require the subcontractor to provide a minimum of fifteen (15) percent of the total grant award from non-federal sources (cash or in-kind). OAA Title III D funding may only be used to support provision of evidence-based mental health services to eligible older adults. Contractors must develop a contribution policy to allow persons served under the Federal Older Americans Act the opportunity to voluntarily contribute to the cost of services provided. The policy must make provisions for confidentiality as well as for procedures to properly account for contributions made by participants. Contributions received must be used to expand and enhance program services. The County reserves the right to utilize other fund sources for these services if they become available. Older Adult Counseling, Family Caregiver Counseling and Evidence-Based Alzheimer’s/Dementia Interventions will be reimbursed at a rate of $150 per hour. Caregiver Support Group Services will be reimbursed at a rate of $175 per group.
