Description
TOWN OF LEESBURG REQUEST FOR PROPOSAL NO 100161-FY07-02 MEDICAL, DENTAL AND VISION BENEFITS Town of Leesburg is requesting proposals to provide Medical, Dental and Vision Benefits for its employees. In addition to the current benefits, the Town is requesting alternate benefit design quotes and the possible inclusion of a Health Savings Account (HSA) plan. A Dental quote with ASO funding is also being requested. The resulting engagement will be a renewable 12 month contract beginning July 1, 2007. The Town of Leesburg will not accept any proposal which incorporates commissions or fees. The solicitation is available for download from the Town’s Bid Board at http://www.leesburgva.gov/services/finance/solicitations/default.aspx Questions must be in writing and sent to Kathy Elgin at kelgin@leesburgva.gov. The RFP number should be referenced in the subject line of all correspondence. Proposals must be received by the Chief Purchasing Officer, 25 W. Market St., Leesburg, VA no later than January, 8, 2007; 5:00 PM
