MEDI-7819 - USA (Claremont, California) - Medi-Cal Outreach, Application and Redetermination Assistance Service - Deadline July 21,2026

(1) Vendor needs to provide medi–cal outreach, application and redetermination assistance service to the government authority located in Claremont, CA.  – To provide Medi–Cal Administrative Activities outreach, education, enrollment and retention assistance to help eligible county residents obtain and maintain Medi–Cal benefits. • Medi–Cal Outreach & Education – Activities that inform individuals or the community about Medi–Cal benefits, services, and eligibility requirements. • Assisting clients in understanding or checking their Medi–Cal status (not making eligibility determinations) • Educating eligible individuals and families about Medi–Cal benefits and services, including agency Enhanced Care Management (ECM) or Community Supports (CS). • Conducting outreach to encourage potential and current beneficiaries to apply for or retain Medi–Cal coverage. • Conducting outreach campaigns targeting potentially eligible populations. • Participating in training on Medi–Cal covered benefits and/or services • Facilitating Medi–Cal Applications  • Providing one–on–one assistance to complete Medi–Cal applications, renewals, and redeterminations. • Assisting clients with gathering required documentation and creating or updating agency accounts for the purpose of establishing or maintaining Medi–Cal status. • Signing up for a Medi–Cal managed care plan (kaiser Partnership, or other available plans) • Participating in training on facilitating Medi–Cal applications, including Medi–Cal eligibility rules and the eligibility process(2) A pre–bid conference will be held on June 23, 2026.(3) All questions must be submitted no later than July 7, 2026.(4) The contract period will be one year.
