RFP No. 23-25 Family Engagement Community Health Education (Home Visits)

Purpose – The objective of this RFP is for the San Bernardino City Unified School District to make award(s), by tasks as specified, to Contractor(s) who meet the requirements to provide home visits and services to families to address social determinants of health and education to increase community health, student health, and academic achievement. By working collaboratively with vendors, the district can create a comprehensive approach to address known barriers to student success while promoting not only academic achievement but health and wellness. Areas of support needed are as follows: The successful Respondent (also referred to as “Contractor” “Offeror” or “Firm” or vendor herein) shall provide all personnel, equipment, transportation, and supplies required to fulfill services needed.
