(1) Vendor needs to provide occupational health screening and examination service to the government authority located in Madison, WI. – Perform occupational and pre–employment health screenings and respirator fitness medical evaluations.– Occupational and pre–employment medical health screening services.– Medical examination and physical fitness evaluation for the use of a respirator.– Respirator protection services for employees and outer agency partners who are assigned to work with clandestine laboratories, arson investigation scenes, tactical operations and performing other work–related tasks which require the use of a personal respiratory protective device.– Perform their job duties without hazard to themselves or fellow co–workers. – Assist the individuals in the maintenance or improvement of their health.– Exam is to assess the employee’s capability to perform their job duties safely and effectively, both physically and mentally.(2) All the questions must be submitted no later than June 19, 2026.
