The City of Tucson, (hereafter referred to as ‘City’) is seeking proposals for the provision of occupational health services and evaluation and treatment of the following services for city employees: GROUP - A: OCCUPATIONAL MEDICINE- Medical Examination, Surveillance and Monitoring, Post- Offer Physicals, DOT Commercial Driver's License Medical Examination, Asbestos Medical Examination, Respirator Use Medical Examination, Drug & Alcohol Testing, Vaccinations, Audiometric Testing, Fitness for Duty evaluations, return to work evaluations, and Workers Compensation Services. GROUP - B: BEHAVIORAL HEALTH SERVICES- Mental Health Counseling/Psychotherapy, Mental Health Chemical Dependency/Addiction Counseling The City intends to award by group and may award to one or more suppliers per group. The offeror may submit to one or both groups.
