(1) Vendor needs to provide caregiver supports program services to the government authority located in Chicago, IL.– Providing assessments, case management, outreach, counseling, support groups, memory cafes, memory and caregiver kits, training and education, and gap–filling services for caregivers.– Increase awareness about the essential value of caregiving and caregiver support services.– Conduct ongoing outreach activities to ensure participation and retention in the program. Outreach efforts may include, but are not limited to community presentations, brochures, flyers, and social media.– Tailored caregiver assessment and referral assessment (case management).– Individual and family counseling.– Support Groups is to provide a safe environment where caregivers can support their peers by exchanging thoughts, ideas, concerns, and coping strategies.– Memory cafe (specialized support groups).– Gap–filling services.– Memory and caregiver kits.– Emergency preparedness and contingency planning.(2) All questions must be submitted no later than June 16,2026.(3) A pre–bid meeting will be held on June 12,2026.(4) The contract period will be for two years.
