Integrated Electronic Health Record (EHR) and Social Service Case Management System - Human Services ARPA

To establish a single contract which may include identified subcontractors to implement and deliver a Human Service and Healthcare information solution that will allow for proactive service delivery, community care coordination, program planning and oversight, and critical decision support to drive community outcomes. The future solution will include, but not be limited to, Electronic Health Record (EHR), social service case management functions, program management functions, eligibility functions, client and provider/partner engagement tools, invoice management, contract and grant tracking, and strong data management, integration, reporting, and analytics. The solution will be a fully integrated approach which combines functionality of a modern Electronic Health Record (EHR) with a scalable, modern social service case management interface that allows for client and data management across a wide range of funded programs. It will have a strong data management, integration platform, and analytic capabilities, and it will allow for efficient and proactive management of clients spanning a range of human service programs, primary care medical services delivery, exchange of information with partners and systems, and predictive analytics to allow for outcomes tracking across programs, budgets, contracts, and grants.
