(1) Vendor needs to provide periodic health assessment modernization capability services to the government authority located in VA.– modernize completion of the periodic health assessment through raw data to decision–ready data, agentic AI, machine learning –based risk scoring, automated notifications, and visualization dashboards, accessible via both web and native or responsive mobile platforms, suitable for operation within a environment, capable of cross domain solution connection and capable of integrating with Health System data sources.– AI– or ML–based risk scoring that triages completed assessments for clinical readiness staff attention, prioritizing review queues by clinical and readiness risk rather than strict first–in–first–out order.– Automated notification and reminder capability spanning service members, clinical readiness staff, and unit commanders, tied to periodic health assessment due dates, overdue status, risk changes and category changes.– Visualization dashboards providing real–time, part–level PHA completion and risk status at the individual, unit, and command level.
