COMPETITIVE CONTRACTING PROPOSAL FOR AREA PLAN CONTRACT FOR ELDERLY, CAREGIVER, AND OLDER RELATIVES

It is the intention of these specifications to provide a variety of services to the elderly, their caregivers, and older relatives providing care to children and adults with a disability funded under the Area Plan Contract (APC). These services will be funded for a period of twelve (12) consecutive months, from January 1, 2025 – December 31, 2025, with the option to extend the term of the contract for two (2) additional one (1) year terms, pending the availability of funding and the approval of the Middlesex County Board of County Commissioners. The services being solicited include Information and Assistance, Information and Assistance in the Field (Outreach), Care Management, Transportation, Assisted Transportation, Assistive Technology, Language Translation & Interpretation, Public Information Services, Home Modification, Social Visiting, Telephone Reassurance, Residential Maintenance/Heavy Chore, Housekeeping, Visiting Nurse, Emergency, Housing Assistance, Support Group, Adult Day Services – Social, Adult Day Services – Medical, Adult Protective Services, Legal Assistance, Physical Health, Mental Health, Physical Activity, Education, Socialization/Recreation, Congregate Nutrition (Regular and Kosher), Home Delivered Nutrition (Regular and Kosher), State Weekend Home Delivered Meals (Regular and Kosher), Nutrition Education, Nutrition Counseling, Portable Nutrition, Groceries, NFCSP Caregiver Support Group, NFCSP Adult Day Services - Social (Respite), NFCSP Trained Volunteer Assistance (Respite), NFCSP Respite (In-Home), NFCSP Respite (Out of Home – Day), and NFCSP ORC Support Group.
