COMPETITIVE CONTRACTING PROPOSAL FOR AREA PLAN CONTRACT FOR ELDERLY, CAREGIVER, AND OLDER RELATIVES

It is the intention of these specifications to provide a variety of services to the elderly and their caregivers serving older adults and individuals with Alzheimer’s and related disorders funded under the Area Plan Contract (APC). These services will be funded for a period of twelve (12) consecutive months, from January 1, 2025 – December 31, 2025, with the option to extend the term of the contract for two (2) additional one (1) year terms, pending the availability of funding and the approval of the Middlesex County Board of County Commissioners. The initial contract was released on September 10, 2024, and is being reissued to obtain additional vendors for specific services. The services being solicited include: Information and Assistance, Transportation, Assisted Transportation, Adult Day Services – Social, Adult Protective Services, Socialization/Recreation, Congregate Nutrition, NFCSP Certified Home Health Aide (Respite), and NFCSP Adult Day Services - Social (Respite).
