(1) Vendor needs to provide family caregiver and evidence–based disease prevention and health promotion services to the government authority located in St. Louis, MO.– Adult family members or other informal caregivers age 18 and older providing care to individuals 60 years of age and older;– Adult family members or other informal caregivers age 18 and older providing care to individuals of any age with Alzheimer's disease and related disorders– Demonstrated through evaluation to be effective for improving the health and well–being or reducing disease, disability and/or injury among older adults.(2) All question must be submitted no later than June 29, 2026.
