CLOSED: Notice of Contract Opportunity: RHTP 4.4b Chronic Disease Management, Remote Patient Monitoring and Assistive Technologies

Purpose: The Nebraska Department of Health and Human Services, Division of Medicaid and Long-Term Care (DHHS), is issuing this Notice of Contract Opportunity for the purpose of identifying eligible and qualified entities to implement the Assistive Technologies component of Rural Health Transformation Program (RHTP) Initiative 4.4b: Chronic Disease Management, Remote Patient Monitoring and Assistive Technologies. This contract opportunity is intended to support technology-enabled safety, cueing, reminder, and environmental supports for older adults and individuals with disabilities so participants can remain safe in their homes and communities. DHHS intends to use an exempted procurement approach for this component based on the specialized nature of the services and technologies involved.
