First 5 Sacramento Health Systems Liaison

The goal of this RFP is to work with an experienced consultant or organization that will serve as the Commission’s liaison to local health systems, health plans, and family service organizations to improve access to services for families . The Health Systems Liaison will conduct an environmental scan, build connections, create strategies to increase collaboration and support closed loop referrals. A maximum of $250,000 is available to implement the services from July 1, 2024 - June 30, 2027. Please see the RFP Specifications Sheet for more information regarding parameters for funding.
