(1) Vendor needs to provide permanent supportive housing services to the government authority located in Sacramento, CA. – Provide on–site project–specific supportive services. – This project will require an organization capable of providing: five full–time supportive services case managers, each operating with a maximum caseload of 20 individuals, one half–time supportive services staff supervisor, and one half–time supportive services coordinator.• Proactive problem–solving to prevent eviction or housing loss which may involve connection to other community services including:– Physical and mental health care– Behavioral health treatment– Peer support activities– Substance use treatment– Connection to VA services– Employment services• Record case documentation, which includes telephone calls/emails, attempts to make contact and client face–to–face interactions.(2) All the questions must be submitted no later than June 26, 2026. (3) Pre–bid meeting will be held on June 29, 2026.
