Functional Assessment

The Jacksonville Transportation Authority (JTA) is seeking proposals from qualified Provider(s) to perform physical and cognitive functional assessments for individuals using and applying for JTA’s paratransit service – JTA Connexion. The Provider(s) selected will provide assessments for new applicants, as well as those whose eligibility will expire during the duration of the contract. JTA anticipates approximately 20-40 hours of services per week but does not guarantee that there will be 2,000 hours per year. The JTA reserves the right to award to more than one Provider based on results of the evaluation phase. Connexion is a door-to-door, ride sharing service for people with disabilities and transportation disadvantaged. JTA is the Community Transportation Coordinator (CTC) for Duval County Florida.
