The purpose of this Request for Proposal is to evaluate the current structure, competitiveness of costs and savings opportunities, capabilities and services available, opportunities for increased employee satisfaction, and compliance of the following components of the current self-funded health benefit programs, which include the following:  Third-party Administrator (TPA) for the claims administration; o Clinical Case Management; o Preferred Provider Organization (PPO) Network Access; o Wellness Program; o Disease Management;  Pharmacy Benefit Management (PBM);  Health Savings Account (HSA) Administration;  Flexible Spending Account (FSA) Administration;  Employee Assistance Program (EAP);  Administrative Services Only (ASO) Vision PPO Plan;  Administrative Services Only (ASO) Dental PPO Plan; and the  Prepaid Dental Plan (Fully Insured).
