(1) Vendor needs to provide caregiver support services to the government authority located in Chicago, IL.• providing assessments, case management, outreach, counseling, support groups, memory cafes, memory / caregiver kits, training & education (including Stressbusting for Family Caregivers), and / or gap–filling services• The goals of Caregiver Support Services are to:– alleviate caregiver stress and social isolation,– empower caregivers to solve or cope with problems caring for care recipient,– improve the caregiver’s health and wellness,– increase awareness about the essential value of caregiving and caregiver support services– increase caregiver’s confidence to continue to provide person–centered care for care recipient,– identify resources and social services to support the caregiver and care recipient– and delay premature institutionalization of the care recipient(2) The contract period will be for one year.
