ADULT DAY REPORTING CENTER AND ACCOUNTABILITY COLLABORATION TREATMENT PROGRAM SERVICES

General work description: This Request for Proposal is to establish an annual contract(s) to provide services at three ADRC program sites and one Accountability Collaboration Training (ACT) site listed below for the Sacramento County Probation Department. The Probation Department is not required to procure all services from any particular vendor and does not guarantee a minimum quantity of services. The Probation Department may choose to award multiple vendors and requires vendors to collaborate and coordinate with each other to meet department requirements. Probation may also require the vendor to collaborate and coordinate with existing programs for department-wide initiatives. The contract(s) resulting from this RFP may be for an initial three-year period commencing with execution of the contract by the Probation Department. For reasons of economy and efficiency, the Probation Department reserves the right to extend contract(s) two additional one-year periods, for a total of five years, upon mutual agreement. Should a contract be awarded as a result of this RFP, continuance of such a contract for the full period specified shall be contingent upon the satisfactory performance of the vendor. Continuing or un-rectifiable performance deficiencies may be cause for the County to cancel any balance of contract. Rates quoted are firm for the initial three-year period. Thereafter, price increases are negotiable based on COLA (Cost Of Living Adjustment), CPI (Consumer Price Index) and must be acceptable to the Probation Department.
