(1) Vendor needs to provide administer a community aging in place program service to the government authority located in Augusta, ME.          – To make safety and functional home modifications and limited repairs to meet the needs of low–income older adult homeowners. – The Program is to enable low–income older adults to remain in their homes through low–cost, low barrier, high impact home modifications to reduce older adults’ risk of falling.– Partner Agencies conduct client intake and assessments, schedule the work for eligible units, and conduct final assessments and inspections.• Client Initial Interview• In–Home Assessment• Home Modification Work• Client Check–In• Final Assessment• Geographic Area to be Served• Key Personnel• Client Information and Unit Production• Current 2026 Community Aging in Place Contract Holders(2) All questions must be submitted no later than June 8, 2026.
