(1) Vendor needs to provide web–based health risk screening tool.– Web–based screening tool to identify and assess health and welfare risks, and support needs across all major life domains.– Includes evaluating health and well–being, safety and behavior, activities of daily living, decision–making capacity, and mental and emotional health.  – Tool will be used by non–clinical staff.– Primary users for screening are case managers.– Guide Appropriate service planning and resource management.(2) All questions must be submitted no later than June 19, 2026.
