Peer Mentoring Services

General work description: The County of Orange (County) Health Care Agency (HCA) is seeking proposals from qualified organizations to provide Peer Mentoring Services. Peer Mentors are individuals with a history of being clients of mental health services; or individuals with lived experience, i.e., living with a relative, significant other, or other support person who is a consumer of mental health services. The primary goal of this program is for Peer Mentors to work in conjunction with Adult and Older Adult Behavioral Health County clinics, County-contracted Behavioral Health outpatient clinics, and County-contracted Full-Service Partnerships to assist clients in developing independent life management and living skills, reducing psychiatric hospitalizations, and supporting their individual recovery goals. In addition, Peer Mentors will be utilized to assist regional hospital providers and the County Crisis Stabilization Unit with linking clients exiting emergency services to appropriate community services. Additionally, Peer Mentors will be embedded with Crisis Assessment Team (CAT) and Mobile Crisis Units to provide rapid response, individual assessment, and community- based stabilization to clients encountered by teams. Peer Mentors will be paired with clients who have been identified as high risk, to help them in successfully transition back into the community after hospitalization.
