(1) Vendor needs to provide cloud–based closed loop referral system. – Provide implementation, hosting, and operation of a Closed Loop Referral System (CLRS).– Implementation plan that supports single–vendor and multi–vendor execution.– System configuration aligned to community workflows.– Solution supports Single Sign–On access.– Solution must provide a comprehensive interactive and real–time resource directory with each service supplier able to update their organization’s service–related information.– Integration with EHRs, HIEs, 211 systems, CBO platforms, and partner systems, when supported and permitted under contractual, operational, technical and applicable legal frameworks. – MPI identity integration. – Data mapping and validation.– Migration testing and reconciliation.– Identity and consent preservation across systems.– Cross–platform workflow validation.– Coordinated go–live strategy.– Maintain data provenance, auditability, and consistency across systems.– Ensure continuity of unique identifiers and MPI–linked identities.– Support cross–platform identity reconciliation.– Maintain and enforce existing patient and client consent across systems.(2) Pre–bid meeting will be held on July 13, 2026. (3) All the questions must be submitted no later than July 17, 2026.
