Description

(1) Vendor needs to provide community breast, cervical and colon prevention program services to the government authority located in Chicago, IL.– Patient Navigation and Breast Cancer Screening: Implement evidence–based patient navigation services to support priority populations across the breast cancer continuum of care, including identifying and addressing barriers to timely diagnostic and treatment services.– Cervical Cancer Prevention: Conduct outreach, education, and navigation to support Pap test completion and follow–up of abnormal cervical screening results, particularly in communities with the lowest cervical cancer screening rates.– Colorectal Cancer (CRC) Screening: Provide education, fecal immunochemical test, fecal occult blood test kit distribution, colonoscopy referral navigation, and follow–up support for abnormal results to reduce the persistent racial gap in colorectal cancer mortality.– Community Health Education: Provide culturally and linguistically appropriate education on cancer prevention, risk–based assessments, and triple negative breast cancer.– Community health education, engagement, and outreach.– Breast screening and diagnostic imaging services.– Linkage to care and financial assistance.(2) All questions must be submitted no later than June 18,2026.
