(1) Vendor needs to provide diabetes self–management education support service to the government authority located in Houston, TX. – Provide Diabetes Self–Management Education and Support (DSMES) services in priority communities, expanding access to evidence–based diabetes education underfunded initiatives.– Deliver Diabetes Self–Management Education and Support (DSMES) programming using approved curriculum.– Recruit and enrol participants from priority populations– Conduct at least five Diabetes Self–Management Education and Support (DSMES) cohorts.– Track attendance, engagement, and outcomes.(2) All the questions must be submitted no later than June 15, 2026.
