Description
This request for quote is for the reproduction of Patient Care Forms for the EMS Department. Each form will need to be on duplication carbonless paper, top sheet shall be white, second sheet shall be yellow. Size shall be 8.5 X 11. Print shall be black. The forms will need to be wrapped in bundles of 200 copies and delivered to our location at 6575 North W Street Pensacola, Fl 32505. We estimate a total purchase of 85,000 copies over the course of the fiscal year in increments of 20,000 per order.
