(1) Vendor needs to provide hepatitis c virus linkage and navigation to care service to the government authority located in Santa Cruz, CA. – Hepatitis C Virus (HCV) and provide linkage and navigation to care with the goal of achieving HCV cure.– Client screening, follow–up with lab–based confirmatory HCV testing, medical visit(s), HCV treatment initiation, prescription pick–up and medication adherence to support treatment completion.– Positive HCV screen to their assigned medical home for lab–based confirmatory HCV testing.– Clients without an assigned medical home will receive medical benefits navigation and support to apply for medical insurance and identify a medical clinic and schedule an initial appointment to receive HCV care.(2) All the questions must be submitted no later than October 16, 2026.
