(1) Vendor needs to provide late life counseling program develop and implement service to the government authority located in Wisconsin. • Late Life Counseling (LLC) is a supportive mental health program designed to reduce stress in older adults and strengthen their coping abilities• The program addresses a wide range of age related emotional, social, and psychological concerns, including isolation and loneliness, depression, physical and cognitive decline, separation anxiety and loss, fear of death, changes in identity and interpersonal roles, age–related prejudice, and financial stress.• LLC also includes outreach and education for community partners. Health and social service professionals, as well as members of the public, are informed about how, when, and where to refer older adults for late life counseling.• Provide surviving caregivers and family members with up to four individualized grief support sessions, which may include direct grief counseling and/or referrals to appropriate grief counseling services, to assist them in adjusting to their loss.• Ensure use of a clinically validated assessment tool in counseling sessions to measure progress and adjust counseling strategies(2) All the questions must be submitted no later than July 29, 2026.
