(1) Vendor needs to provide grant evaluation services to the government authority located in Arlington, VA. • Developing and refining the project evaluation plan • Monitoring implementation fidelity to the System of Care model • Supporting continuous quality improvement • Developing data dashboards and performance reports • Monitoring progress toward all grant goals and objectives • Assisting with agency performance measurement and reporting • Conducting process, outcome, and implementation evaluations • Designing and administering surveys • Conducting qualitative interviews and focus groups • Preparing annual evaluation reports • Participating in Governance Board meetings and meetings • Presenting findings to Horizon leadership, and community stakeholders • Assisting with dissemination through conference presentations and peer–reviewed publications (2) All questions must be submitted no later than August 16, 2026.(3) The contract period will be four years.
