(1) Vendor needs to provide physical therapy services to the government authority located in Sacramento, CA.– Care provided shall be appropriate to the resident's condition and consistent with the professional knowledge, judgment and skills of a licensed Physical Therapist.– Conduct an initial assessment or evaluation for each authorized referral to determine the resident'sfunctional status, identify the nature and extent of impairments, develop an individualized treatment plan based on the assessment findings and communicate the treatment plan to the resident, interdisciplinary team members and other appropriate healthcare professionals.– Provide education and instruction regarding therapy services, treatment plans, safety measures,equipment use and home exercise programs to residents, family members, caregivers and Home staff, as appropriate.(2) All question must be submitted no later than September 3, 2026.
