(1) Vendor needs to provide digital closed–loop referral platform. – Implementation, and ongoing management, maintenance, and operations services for a digital closed–loop referral platform to strengthen communication and collaboration among health care, mental health, substance use–disorder, and social service providers.– System should ensure patients successfully connect to needed community supports, such as housing, food assistance, and transportation.– Enabling real–time data sharing and tracking of referral outcomes, the platform should reduce duplication of services, improve accountability, and promote cost efficiency while ensuring more seamless, person–centered care across health and social service systems.(2) All the questions must be submitted no later than June 9, 2026.
