(1) Vendor needs to provide community–clinical linkages implementation support service to the government authority located in Mississippi.– In–state school of pharmacy to partner with providing implementation support for the community pharmacy program.– Providing pharmacy education, training, and experiential learning opportunities for PharmD students and pharmacy residents.– Engaging in community–based or population health initiatives, particularly those focused on chronic disease prevention and management.– Conducting or contributing to implementation, evaluation, or quality improvement activities in healthcare or public health settings.– Develop and implement workforce development initiatives, including student and resident training experiences aligned with rural health and population health priorities.– Track and monitor clinical measures shown to improve health and wellness and health care quality within populations with and at the highest risk of hypertension and high cholesterol.– Link community resources and clinical services that support comprehensive bidirectional referral and follow–up systems aimed at mitigating social services and support needs for optimal health outcomes.– Support workforce development through pharmacist, pharmacy technician, and pharmacy student and resident learning experiences focused on public health, rural health, and population health strategies.– Strengthen community–clinical linkages between community pharmacies, healthcare systems, and community organizations.– Support community outreach and cardiovascular screening activities conducted in collaboration with staff and partners.(2) All questions must be submitted no later than July 31, 2026.
