(1) Vendor needs to provide anesthesiologist services to the government authority located in Brentwood, NY.– License and registration.– English or Bilingual (English and Spanish) language.– Privileges include assessment of patients and induction of Anesthesia to patients undergoing treatments.– Direct contact between medical clinic personnel and scheduled patients.– Consultation with referring prescriber as clinically indicated.– Documentation of evaluation and assessment of patients along with procedure notes will be givenin clear concise form and provide detailed information on the patient as per CMS Standards.– Participation in trainings required for a consultant.(2) All the questions must be submitted no later than July 30, 2026.(3) Pre–bid conference will be held on July 22, 2026.
