(1) Vendor needs to provide health commons services to the government authority located in Los Alamos, NM. – Be licensed in the State to provide medical and pharmacy services, possess liability and malpractice insurance at levels adequate to cover all exposures, and have experience providing health and wellness services.– Provide medical services related to family planning, including licensed practitioner consultations, examinations, prescriptions and continuing supervision, laboratory services, contraceptive supplies, and referrals to other medical facilities when medically indicated.– Provide sexually transmitted infection screening for all family planning clients and others upon request, including examinations, laboratory services, treatment of infections within the clinic's capabilities, referrals to appropriate providers for conditions requiring a higher level of care, and partner services.– Provide services in a manner that protects the dignity, privacy, and confidentiality of each individual.– Provide services without regard to religion, race, color, national origin, disability, age, sex, number of pregnancies, or marital status.– Provide services on a sliding–fee scale to low–income or uninsured patients.(2) All the questions must be submitted no later than October 14, 2026.
