HEALTH-11158 - USA (Maryland) - Independent Evaluation Consultant Service - Deadline November 24,2026

(1) Vendor needs to provide independent evaluation consultant service to the government authority located in Maryland. – To establish an integrated, technology–enabled system of care that connects residents to primary care, behavioral health, oral health, public health services, wellness supports, emergency response systems, and resources that address health–related social needs.– The initiative centers on a Community–Integrated Care Coordination system using embedded care coordinators, closed–loop referral technology via the Health & Wellness Portal, digital health supports, mobile integrated health outreach, and multidisciplinary care planning.– To develop and implement a public health mixed–methods evaluation and continuous quality improvement approach to assess both the implementation and outcomes.– The evaluation is expected to draw on multiple quantitative and qualitative data sources, including Health & Wellness Portal analytics, agency resources, participant and partner feedback, and other available data.(2) A pre–bid meeting will be held on November 9, 2026.(3) The contract period will be one year.
