(1) Vendor needs to provide clinical quality improvement technical assistance provider services to the government authority located in Utah.• To clinics working on clinical quality improvement projects related to improving hypertension control and decreasing the risk of cardiovascular disease, diabetes prevention and management, and tobacco cessation.• Clinic–level quality improvement projects focused on increasing the enrollment into the national diabetes prevention program, diabetes self–management education and support programs.• The clinical sub–workgroup who are from clinics within geographic areas identified by census tract areas that fall within the 40th percentile of hypertension and its comorbidities (high cholesterol, stroke, and coronary heart disease).(2) All questions must be submitted no later than Jul 15th 2026.
