FMD 25-022 Emergency Medical Transportation

The City of Avondale (the “City”) is issuing this Request For Proposals (this “RFP”) seeking proposals (“Proposals”) from qualified, licensed firms (“Vendors”) interested in providing professional services consisting of Brief Project Description (fill in the blank) (the “Services”), to meet the needs of the Avondale Fire and Medical Department (“AFMD”) and Avondale Police Department ("APD"). This proposal also incorporates the rules and regulations of the Arizona Department of Health Services governing the provision of Emergency Transportation services. Basic Life Support (“BLS”) and Advanced Life Support (“ALS”) staffing in conjunction with stand-by ambulance services may be requested for Special Events as determined to be necessary by the City.
