(1) Vendor needs to provide surveyor services to the government authority located in Wisconsin.– Surveyors are needed to initiate an investigation of complaints for long–term care (LTC) and non– long–term care (NLTC) facilities and recertifications. – Solicitation is divided into two (2) Lots and will result in up to three (3) separate contracts for each Lot.– Lot 1 – Surveyor services for long–Term care (LTC) facilities.– Lot 2 – Surveyor services for non–long–term care (NLTC) facilities.(2) All the questions must be submitted no later than November 4, 2026.(3) The contract period will be for one year.
