Description
This RFP is the process by which the County will solicit proposals for the purpose of entering into a contract with selected respondents for a 12-month period from July 1, 2024 through June 30, 2025, with the possibility of an annual contract renewal and/or extension for three (3) additional years based upon satisfactory performance and available funding. The County of Solano, Department of Health and Social Services, Napa/Solano Area Agency on Aging (N/S AAA) located at 275 Beck Ave. Fairfield, CA 94533, hereinafter referred to as the “County,” intends to secure contracts with qualified proposers to provide Supportive Services, Family Caregiver Services, and Health Promotion Services to older adults and adults with disabilities. Non-profit organizations, health care systems, for-profit organizations, educational institutions, and government entities are eligible to apply for multiple services covering one or both counties. The objectives should state the target quantities and match the program services as proposed. At a minimum, the proposal must include: Define the need based on data Number of service units Number of unduplicated clients Service delivery methods Outreach plan Coverage area For Information and Assistance, include service locations and hours, client follow-up methods, and partner resource list Referral process Outcome Objectives Contractor shall develop specific outcome objectives that demonstrate and measure the health impact, outcomes, or results of services. Both quantitative and qualitative analysis shall be applied to measure program efficiency and effectiveness. The outcome objectives must be specified for each service. Objectives must be specific, measurable, achievable, relevant, and time bound. Reporting Requirements Contractor shall provide various reports during the term of the contract and shall: Facilitate the California Department of Aging (CDA) Security Awareness Training to Program Staff on an annual basis and maintain evidence of staff completion of this training. Comply with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) privacy and security rules to the extent applicable. Develop and deliver ad hoc reports as requested.
