(1) Vendor needs to provide memory network program services to the government authority located in Honolulu, HI.– Memory network program is a statutorily established statewide dementia care model designed to improve early detection, diagnosis, care planning, and access to treatment for individuals living.– These funds will be used to establish memory clinics, dementia care specialists, and develop a coordinated statewide network linking primary care, specialty care, and home and community–based services.– Memory network program serves as foundational infrastructure to ensure equitable, statewide access to dementia care, with a focus on early intervention and improved care coordination.(2) Pre–bid meeting will be held on August 4, 2026.
