(1) Vendor needs to provide strategic and housing navigation consulting services. – The Consultant is to work closely with agency to meet community needs and promote long–term stability, including permanent affordable housing solutions for the participants/applications of the various housing programs.– The Consultant is responsible for casework services that will assess housing needs, make appropriate referrals, and provide information regarding available housing units with the goal of obtaining permanent affordable housing for agency clients.– Case management services can include, but not limited to, credit repair, securing financial resources, identifying community partners to provide support services, and obtaining needed household items (e.g., furniture) as approved.– Follow–up with clients– Maintains records, files, reports, and other documentation for each client referred for services.(2) All the questions must be submitted no later than September 10, 2026.
