EXTRA-74700 - USA (Phoenix, Arizona) - Re-entry Transitional Support Service - Deadline August 27,2026

(1) Vendor needs to provide re–entry transitional support service to the government authority located in Phoenix, AZ.– The Solicitation includes two distinct programs: The agency Program and the Set for Success Transition Program.– Provide evidence–based programing, case management, housing stabilization, behavioral health services, transition planning, and supportive services designed to promote successful community reintegration, improve continuity of care, reduce recidivism, and enhance public safety.– Provide a comprehensive, integrated recovery program that includes, at a minimum:– Three (3) prepared nutritional balanced meals per day in accordance with dietary guidelines to support the health and well being of all clients.– Behavioral health services;– Case management and care coordination;– Substance abuse treatment;– Housing stabilization support;– Employment and education services;– Access to medical and mental health care;– Life skills and pro–social development; and– Transportation Services.(2) The contract period will be for one year.(3) Pre–bid meeting will be held on August 18, 2026.
