(1) Vendor needs to provide health care supports and services to the government authority located in Hartford, CT.– Health Care and Support Services (HCSS) Program to ensure that medically underserved, low–income individuals with HIV have access to essential care and services.– Implement a program to either serve as one of two separate geographical planning region hubs to provide access to a full range of core and support services, or to serve as one statewide coordinator.– Services aim to improve retention in care, achieve viral load suppression, reduce health disparities, and contribute to ending the HIV epidemic.– Improve viral suppression rates.– Enhance linkages to core and support services.– Early intervention services.– Health insurance premium and cost sharing assistance– Medical nutrition therapy– Mental health service– Oral health care– Outpatient and ambulatory health services– Substance abuse outpatient care(2) All questions must be submitted no later than July 22,2026.(3) A pre–bid meeting will be held on July 29, 2026.(4) The contract period will be for three years.
