(1) Vendor needs to provide cancer survivorship evaluation services to the government authority located in Mississippi.A. Assist in scheduled survivorship planning meetings for the staff of the Comprehensive Cancer Control Program.B. Be available by phone bi–weekly to assist the Comprehensive Cancer Control Program personnel in accomplishing the goals of the survivorship section of the Comprehensive Cancer Control Program.C. Be available to develop and deploy cancer survivors and caregiver's surveys and needs assessments via the Qualtrics platform at the request of the Comprehensive Cancer Control Program administration.D. Help identify existing data sources or tools for the survivorship evaluation.E. Monitor progress towards developing reports and presentations on cancer survivorship.
