HEALTH-10972 - USA (New York) - Neighbourhood Navigator Initiative Services - Deadline November 24,2026

(1) Vendor needs to provide neighbourhood navigator initiative services to the government authority located in New York.• Building trusting relationships between navigators and people in need of support who are homeless or have behavioural health needs, • Increasing access to and use of existing resources and services by people in need of support who work with navigators, • Creating ecosystems of care coordination among local service providers, and • Responding to community concerns as a known resource and partner. – Provide consistent, community–based engagement, respond to immediate needs, and over time serve as trusted bridges to healthcare, behavioural health treatment, housing, benefits, and other resources. Navigators will accompany participants to appointments when appropriate and facilitate warm handoffs to partner providers.– Navigators will work in teams with clinical staff and seek to meet basic needs, including food, hot coffee, blankets, warm clothing, fentanyl test strips, naloxone, medical supplies, personal protective equipment, and other immediate needs as appropriate.(2) Pre–bid meeting will be held on October 29, 2026.(3) All the questions must be submitted no later than October 21, 2026.
